Adult Care Management

Guardian Care and Geriatric Services provides care management services to elderly individuals or couples and to adults with disabilities. Care managers assist individuals with long-term care planning by assessing each situation and coordinating needed services. 

• The initial assessment is an extensive evaluation of the health, social/personal situation, and residential setting of a person. The care manager is often hired by a concerned out-of-town family member.

• Ongoing services may include any or all of the following areas:

  • Reviewing health insurance claims (Medicare, Medigap, long-term care, etc.) and communicating with medical care providers to resolve issues

  • Visiting with a client at home, assisted living facility, nursing home, or hospital.

  • Assisting as a liaison between care providers and a client

  • Coordinating and supervising private caregivers whether in-home or in a facility setting

  • Managing finances and paying bills

  • Applying for public or private benefits

  • Managing property and arranging for necessary repairs

  • Assisting clients with residence changes and the transition process
