Guardian
Care and Geriatric Services provides care management
services to elderly individuals or couples and to adults with
disabilities. Care managers assist individuals with long-term
care planning by assessing each situation and coordinating
needed services.

The
initial assessment is an extensive evaluation of the
health, social/personal situation, and residential setting
of a person. The care manager is often hired by a concerned
out-of-town family member.

Ongoing
services may include any or all of the following areas:

Reviewing
health insurance claims (Medicare, Medigap, long-term
care, etc.) and communicating with medical care
providers to resolve issues

Visiting
with a client at home, assisted living facility, nursing
home, or hospital.

Assisting
as a liaison between care providers and a client

Coordinating
and supervising private caregivers whether in-home or
in a facility setting

Managing
finances and paying bills
-
Applying
for public or private benefits
-
Managing
property and arranging for necessary repairs
-
Assisting
clients with residence changes and the transition
process
